IRC 21 VOLUNTARY RECALL NOTICE

Fred Blank - Tri State Theatre Supply

TO:             Purchasers of IRC-21 units

FROM:        Ultra-Stereo Labs, Inc. (USL)

DATE:         November 19, 2010

Re.:             IRC-21/UPC-21 (manufactured between May 2006 and August 2008)

We have recently been informed that IRC-21/UPC-21 units in two theaters have overheated, causing damage to the units and/or their surroundings.  Preliminary investigations by USL and at least one government agency have failed to determine the cause of the overheating.  While there are thousands of these units operating normally worldwide, USL is conducting a voluntary recall of the units out of an abundance of caution.

The recall applies to units manufactured in between May 2006 and August 2008 and requires that the following steps be taken:

     1. Immediately unplug any units in your possession from their power sources;
     2. Contact our Customer Service Department at 1-805-549-0161 or sales@uslinc.com

         to get a tracking number to return the units to us at our expense;
     3. Return the units to us, and we will rework them and return them to you

         as soon as possible;
     4. While you are waiting for your units to be returned, you will need to comply with

         Americans with Disabilities Act requirements regarding repairs to assistive listening

         devices by including a notification on your pre-recorded telephone messages stating

         that your assistive listening devices are not functioning and an estimated date by

         which they will be functioning.  Our best estimate of the time it will take us to rework

         the units is 6 weeks, so you should use that time frame to estimate the date by which

         the devices will be functioning.  Although the sources we reviewed only referenced

         including the notification on your pre-recorded telephone messages, it would be wise

         to include it on your website as well.  Please note that your failure to include the

         notification on your pre-recorded telephone messages could require you to issue free

         passes to a future showing, a refund of any money paid for tickets, and a verbal or

         written apology, to any person who cannot attend a showing because your assistive

         listening devices are not functioning.

We are continuing our investigation, and we will provide you with additional information as it becomes available.

We continue to believe in the IRC 21 and its many years of good service to the theater industry, but we also believe in making sure our products are as safe as possible.

Thank you for your immediate attention to this important matter.

Sincerely,

USL, Inc. Sales Department     sales@uslinc.com
